If massage therapy is prescribed to you by a health care provider, massage therapists don’t have to charge you sales tax on your massage session.
The health care provider has to give you a written prescription which will be kept in your file The prescription should have the following information:
- Name, address and license number of the health care provider.
- Your name and address.
- Reason for prescription ( diagnostic code, pain management, stress reduction, muscle tightness etc…)
- Frequency of the massage sessions.
- How long the prescription is good for.
I am always happy to talk with your Doctor or his staff if they have any questions, or I can email them a form.
This prescription is not for insurance purposes but to be presented to the Sales Tax administration in case of an audit.
